Radium-223 dichloride + docetaxel is an investigational combination therapy for the treatment of bone-predominant metastatic castration-resistant prostate cancer (mCRPC). **Radium-223 dichloride** is an alpha particle-emitting radiopharmaceutical that specifically targets areas of increased bone turnover, such as bone metastases, delivering localized radiation and resulting in double-strand DNA breaks in adjacent tumor cells and the bone microenvironment. **Docetaxel** is an antimicrotubule cytotoxic chemotherapy agent that inhibits cell division by promoting the assembly and stabilization of microtubules and ultimately inducing apoptosis in rapidly dividing cells, including cancer cells. The combination is designed to dually target tumor cells and osteoblastic bone lesions. Phase 1/2 studies have shown the combination can be administered safely at radium-223 55 kBq/kg every 6 weeks (5 doses) plus docetaxel 60 mg/m^2 every 3 weeks (10 doses). Exploratory efficacy data suggest the combination may offer enhanced antitumor activity compared to docetaxel alone, with durable suppression of prostate-specific antigen and bone formation markers, as well as acceptable safety[1][2][7].
