Overview

ranibizumab + PASCAL laser photocoagulation is a combination therapy regimen evaluated in the IRALA clinical trial by the University of Sao Paulo for the treatment of proliferative diabetic retinopathy (PDR). This approach integrates the pharmacological action of ranibizumab, a recombinant humanized monoclonal antibody fragment (Fab) that binds to and inhibits vascular endothelial growth factor A (VEGF-A), with the mechanical effects of panretinal photocoagulation (PRP) delivered via the PASCAL (Pattern Scan Laser) system. Ranibizumab reduces vascular permeability and suppresses the growth of abnormal new blood vessels by blocking the interaction of VEGF-A with its receptors, VEGFR-1 and VEGFR-2. The PASCAL system utilizes a semi-automated scanning method to apply patterns of laser spots to the peripheral retina, which reduces the metabolic demand of the ischemic tissue and leads to the regression of neovascularization. This combination therapy is designed to leverage the rapid onset of anti-VEGF agents with the durable, long-term effects of laser photocoagulation while minimizing the treatment burden and patient discomfort associated with traditional single-spot laser techniques.
