Overview

**Ranpirnase + doxorubicin hydrochloride** is a combination of two drugs used primarily in clinical research for treating malignant mesothelioma and other solid tumors. **Ranpirnase** is a novel ribonuclease enzyme that preferentially degrades transfer RNA (tRNA), inhibiting protein synthesis and acting as both a cytostatic and cytotoxic agent. **Doxorubicin hydrochloride** is a well-established anthracycline chemotherapeutic that intercalates DNA and inhibits topoisomerase II, leading to DNA strand breaks and inhibition of replication. The combination has shown synergistic antitumor effects, especially in preclinical and clinical studies of mesothelioma, with improved median survival over doxorubicin alone. Ranpirnase’s unique mechanism allows it to be combined with doxorubicin without increasing doxorubicin-related side effects. Ranpirnase is also known by its trade name Onconase. Development was led primarily for second-line therapy in mesothelioma, where no standard of care existed, until pemetrexed plus platinum agents became first-line. Clinical trials suggest that the combination therapy stabilizes progressive disease and may extend survival over monotherapy[1][2][3][4][5].
