Overview

Rapamycin + vildagliptin is a combination of two small molecule drugs, each with distinct mechanisms of action. Rapamycin (also known as sirolimus) is an mTOR inhibitor primarily used for its immunosuppressive and anti-proliferative effects, while vildagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that enhances incretin hormone activity to improve glycemic control. This combination has been investigated in clinical trials for its potential to reduce insulin requirements and modulate immune responses in patients with long-standing type 1 diabetes. In a phase 2 trial, the combination reduced insulin requirements but did not restore β-cell function[1]. The rationale behind this pairing lies in rapamycin’s immunomodulatory properties and vildagliptin’s ability to enhance endogenous insulin secretion by preserving incretin hormones.
