Overview

This refers to a combination therapy using a renin-angiotensin system (RAS) antagonist—such as an angiotensin-converting enzyme inhibitor (ACE inhibitor), angiotensin II receptor blocker (ARB), or direct renin inhibitor—with a beta-adrenergic receptor antagonist (beta-blocker). Both drug classes are antihypertensive agents but act via different mechanisms. RAS antagonists inhibit the renin-angiotensin system, reducing vasoconstriction and aldosterone-mediated volume expansion, while beta-blockers decrease heart rate and cardiac output by blocking adrenergic stimulation of the heart. The combination is used in certain patients with hypertension, heart failure with reduced ejection fraction (HFrEF), or other cardiovascular conditions where dual blockade may provide additive benefits for blood pressure control or organ protection. However, this combination is not routinely recommended for all patients due to potential risks such as hypotension, bradycardia, and renal impairment; its use should be individualized and closely monitored[1][5][6][8].
