Overview

A combination therapy using rasagiline (a selective monoamine oxidase type B inhibitor) with antidepressants for patients with Parkinson's disease who also have depression. Depression is a common comorbidity in PD, affecting approximately 40-50% of patients. While current rasagiline labeling generally advises against coadministration with antidepressants due to potential drug interactions, clinical evidence suggests this combination may be used safely in certain circumstances and may provide benefits for non-motor symptoms. Studies have shown that rasagiline in combination with antidepressants can reduce the worsening of fatigue and daytime sleepiness scores. The combination has also been associated with reduced worsening of a range of non-motor symptoms in preliminary analyses of patients with de novo (newly diagnosed) Parkinson's disease. Despite warnings about potential interactions, clinical trials including TEMPO and LARGO studies permitted limited use of certain antidepressants with rasagiline, including amitriptyline (\u226450 mg/day), trazodone (\u2264100 mg/day), citalopram (\u226420 mg/day), sertraline (\u2264100 mg/day), and paroxetine (\u226430 mg/day), with no adverse interactions reported. The primary concern is the risk of serotonin syndrome, a potentially life-threatening condition. Current rasagiline labeling warns against concomitant use with many classes of antidepressants, including SSRIs, SNRIs, tricyclic, tetracyclic, and triazolopyridine antidepressants. Serotonin syndrome has been reported with the combined use of antidepressants and MAO inhibitors, including selective MAO-B inhibitors like rasagiline. Tertiary sources warn that the combination of MAO-B inhibitors and SSRIs can result in increased serotonergic effects, potentially leading to serotonin syndrome. Clinical case evidence exists of successful use of rasagiline in combination with two antidepressants (venlafaxine and bupropion) in a patient with Parkinson's disease and major depressive disorder, suggesting it may be used safely with proper clinical judgment and close monitoring.
