RBS2418 + pembrolizumab is an investigational combination therapy being evaluated for advanced, unresectable, recurrent, or metastatic solid tumors. RBS2418 is a potent, selective oral inhibitor of ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1), an enzyme that hydrolyzes extracellular cGAMP and ATP and generates adenosine, a mediator of tumor immune suppression. By inhibiting ENPP1, RBS2418 increases levels of cGAMP and ATP in the tumor microenvironment and reduces immunosuppressive adenosine, activating antigen-presenting cells and enhancing T-cell infiltration to promote anticancer immunity. Pembrolizumab is a monoclonal antibody targeting programmed cell death protein 1 (PD-1), acting as a checkpoint inhibitor to enhance antitumor immune responses. The combination of RBS2418 and pembrolizumab is being tested in Phase 1a/b trials for safety, pharmacokinetics, and preliminary anti-tumor activity, including in heavily pretreated patient populations.[1][2][3][4][6]
