Realgar-Indigo Naturalis Formula (RIF) + cisplatin is a combination therapy being investigated for the treatment of osteosarcoma. RIF is a traditional Chinese medicine formula primarily composed of realgar (containing arsenic sulfide, $As_4S_4$), Indigo naturalis, and Salvia miltiorrhiza. While RIF is traditionally used and approved in China for acute promyelocytic leukemia (APL), researchers at The First Affiliated Hospital of Sun Yat-sen University are exploring its synergistic potential with cisplatin in solid tumors like osteosarcoma. The mechanism of action involves a dual approach: cisplatin induces DNA cross-linking and damage, while the arsenic components of RIF promote the generation of reactive oxygen species (ROS), inhibit thioredoxin reductase, and trigger mitochondrial-mediated apoptosis. This combination is designed to overcome chemoresistance and enhance the pro-apoptotic effects of standard chemotherapy in bone malignancies.
