Overview

Recombinant factor VIIa + tranexamic acid is a combination hemostatic regimen investigated by Sheba Medical Center for the management of patients with severe Factor XI deficiency (hemophilia C) undergoing surgery. Recombinant factor VIIa (rFVIIa), originally developed by Novo Nordisk under the brand name NovoSeven, is a biologic bypassing agent that promotes hemostasis by activating coagulation factor X and factor IX, thereby enhancing thrombin generation at sites of tissue injury. Tranexamic acid is a small molecule antifibrinolytic that stabilizes formed clots by competitively inhibiting the activation of plasminogen. This specific protocol utilizes a single low dose of rFVIIa (20 ug/kg) in conjunction with a multi-day course of tranexamic acid to provide effective bleeding prophylaxis while minimizing the need for blood products like fresh frozen plasma.
