Overview

Red blood cells + tranexamic acid + fibrinogen concentrate is a combination therapeutic regimen being investigated for the pre-hospital management of severe trauma and hemorrhagic shock. This protocol, evaluated in a pilot trial led by Cristina Martinez, involves the early administration of three critical components: red blood cells to restore oxygen delivery to tissues, tranexamic acid to act as an antifibrinolytic by inhibiting plasminogen activation, and fibrinogen concentrate to provide a concentrated source of clotting factor I. The goal of this intervention is to mitigate trauma-induced coagulopathy and improve survival by initiating definitive hemostatic resuscitation before the patient reaches the hospital, contrasting with traditional crystalloid-heavy resuscitation strategies.
