REMD-477 + metformin is a combination regimen of volagidemab (REMD-477), a fully human monoclonal antibody targeting the glucagon receptor, and metformin, a classic oral antihyperglycemic agent. REMD-477 blocks the glucagon receptor, thereby inhibiting glucagon-mediated hepatic glucose production, which is implicated in hyperglycemia in diabetes. Volagidemab was originally developed at Amgen and licensed to REMD Biotherapeutics; it has been evaluated in phase 1 and phase 2 studies for both type 1 and type 2 diabetes, showing reductions in insulin requirements and improvements in glycemic control. Metformin, a small molecule, acts primarily by reducing hepatic glucose production and increasing insulin sensitivity, and is considered the first-line oral treatment for type 2 diabetes.
