Overview

Remifentanil + lidocaine is a combination of two pharmaceutical agents commonly used in anesthesia and perioperative pain management. Remifentanil is an ultra-short-acting synthetic opioid analgesic that acts as a potent agonist at the µ-opioid receptor, providing rapid-onset and short-duration analgesia. It is primarily used for induction and maintenance of general anesthesia, conscious sedation, and postoperative pain control[1][4][6]. Lidocaine is a widely used local anesthetic with additional properties including suppression of cough reflexes, analgesia, anti-hyperalgesia, and anti-inflammatory effects[5]. When administered together intravenously during surgical procedures or intubation/extubation events, lidocaine can reduce the required dose of remifentanil to suppress airway reflexes such as coughing while also decreasing the risk of respiratory depression associated with higher opioid doses[5]. This combination leverages both central (remifentanil) and peripheral (lidocaine) mechanisms to optimize patient comfort and safety during anesthesia.
