Overview

Remimazolam + flumazenil refers to a pharmacological regimen involving the administration of remimazolam, an ultra-short-acting intravenous benzodiazepine, followed by its reversal with flumazenil, a selective benzodiazepine receptor antagonist. Remimazolam functions as a positive allosteric modulator at the GABA-A receptor, providing rapid-onset sedation and anesthesia with a predictable offset due to its metabolism by tissue esterases. Flumazenil is subsequently administered to competitively inhibit the benzodiazepine binding site on the GABA-A receptor, promptly reversing residual sedation. This specific combination is being investigated by Seoul National University Hospital in clinical trials to evaluate its efficacy in reducing the incidence and severity of emergence agitation in patients undergoing nasal surgery, comparing it against propofol-based total intravenous anesthesia (TIVA).
