Overview

This therapeutic intervention involves the sequential administration of remimazolam and flumazenil. Remimazolam is an ultra-short-acting small molecule benzodiazepine that acts as a positive allosteric modulator at the GABA-A receptor, providing rapid-onset sedation and anesthesia with a predictable offset due to its metabolism by non-specific tissue esterases. Flumazenil is a competitive benzodiazepine receptor antagonist used to promptly reverse the sedative effects of remimazolam. The Medical University of Gdansk is evaluating this combination in a Phase 4 clinical trial to assess its impact on cognitive outcomes in elderly patients. The primary objective is to determine if the active reversal of remimazolam-induced sedation with flumazenil can reduce the incidence of postoperative delirium (POD) and postoperative neurocognitive disorder (PND) following hip arthroplasty under spinal anesthesia.
