Repaglinide + insulin refers to the concurrent use of two antidiabetic agents for the management of type 2 diabetes mellitus. Repaglinide is an oral antihyperglycemic agent from the meglitinide class that stimulates pancreatic beta cells to release insulin by closing ATP-dependent potassium channels, thereby increasing endogenous insulin secretion in a glucose-dependent manner. Insulin, administered exogenously (often as long-acting formulations like insulin glargine), acts as a replacement or supplement to endogenous human insulin, facilitating cellular glucose uptake and reducing blood glucose levels. The combination is used when monotherapy with either agent does not achieve adequate glycemic control. Clinical studies indicate that combining repaglinide with basal insulins such as glargine can improve both fasting and postprandial blood glucose control, reduce required doses of each drug, and lower the risk of hypoglycemia compared to higher-dose monotherapy[6]. This regimen is primarily indicated for adults with type 2 diabetes who require intensified therapy.
