Retlirafusp alfa + CAPOX is a combination therapy being investigated for the treatment of locally advanced rectal cancer. Retlirafusp alfa (IBI363) is a first-in-class bispecific antibody-cytokine fusion protein (immunocytokine) that simultaneously targets the Programmed Cell Death Protein 1 (PD-1) and the Interleukin-2 (IL-2) receptor. It is designed to selectively deliver IL-2 to PD-1-expressing T cells, thereby enhancing anti-tumor immune responses while minimizing systemic toxicity associated with high-dose IL-2. CAPOX (also known as XELOX) is a standard chemotherapy regimen comprising capecitabine, an oral prodrug of 5-fluorouracil, and oxaliplatin, a platinum-based antineoplastic agent. In clinical trials, this combination is often evaluated in a neoadjuvant setting alongside radiotherapy to improve pathological complete response rates and facilitate organ preservation in rectal cancer patients.
