rFVIIa + FEIBA is a combination therapy consisting of recombinant activated factor VII (rFVIIa; commonly known by the brand name NovoSeven) and Factor Eight Inhibitor Bypassing Activity (FEIBA), an activated prothrombin complex concentrate. Both agents are used as bypassing therapies for patients with hemophilia A or B who have developed inhibitors to factor VIII or IX. The combination is sometimes employed in cases where bleeding episodes are refractory to either agent alone. Mechanistically, rFVIIa directly activates the coagulation cascade at the site of injury by activating factor X on the surface of activated platelets independent of tissue factor, while FEIBA provides a mixture of vitamin K-dependent clotting factors (mainly non-activated II, IX, X and activated VII) that can bypass the need for factors VIII or IX in coagulation. The combined use has been shown to have a synergistic effect on thrombin generation but carries an increased risk of thrombosis; thus it is generally reserved for life-threatening or refractory bleeding episodes in inhibitor-positive hemophilia patients[2][5][6].
