The combination of recombinant human insulin-like growth factor-1 (rhIGF-1) and 17-beta estradiol is a therapeutic approach that has been studied for treating bone impairment in adolescent girls and young women with anorexia nervosa (AN). This combination therapy involves transdermal delivery of 17-beta estradiol along with subcutaneous injections of rhIGF-1. ## Description This combination therapy consists of: - Transdermal 17-beta estradiol (0.1 mg/day delivered via patch) - Recombinant human insulin-like growth factor-1 (rhIGF-1) administered subcutaneously at 30 mcg/kg twice daily The treatment regimen typically includes progesterone (100 mg daily for the first 10 days of each month) to prevent endometrial hyperplasia from unopposed estrogen. The rhIGF-1 dosage is adjusted to maintain levels in the upper half of the normal pubertal range. Clinical studies have investigated this combination for treating bone impairment in adolescent girls with anorexia nervosa, but research has found no additive benefit of rhIGF-1 administration over transdermal estrogen replacement alone in this population[1][6].
