Ridaforolimus + dalotuzumab is an investigational combination therapy for cancer. Ridaforolimus is a non-prodrug analog of rapamycin and acts as a potent, selective inhibitor of the mammalian target of rapamycin (mTOR), a serine/threonine kinase involved in regulating cell growth, proliferation, and survival. Dalotuzumab is a humanized IgG1 monoclonal antibody targeting insulin-like growth factor 1 receptor (IGF1R), blocking ligand binding and promoting receptor internalization and degradation. The rationale for combining these agents stems from the compensatory activation of IGF1R signaling upon mTOR inhibition; dual blockade with ridaforolimus and dalotuzumab more effectively inhibits the PI3K/AKT/mTOR pathway, preventing feedback activation of AKT and enhancing antitumor activity. This combination has been studied in advanced solid tumors including breast cancer, sarcomas, neuroblastoma, rhabdomyosarcoma, adrenal cortical carcinoma, and other pediatric cancers[1][2][3][5][6].
