Overview

This is a fixed-dose oral capsule combination of three widely marketed antibacterial drugs: clarithromycin, clofazimine, and rifabutin. The combination is designed to treat nontuberculous mycobacterial (NTM) infections, particularly Mycobacterium avium complex (MAC) infections. The triple combination is formulated to mitigate the emergence of macrolide resistance that is often observed with macrolide monotherapy. Each component contributes to the overall antimycobacterial effect: - Clarithromycin is a macrolide antibiotic that inhibits protein synthesis by binding to the 50S ribosomal subunit - Rifabutin is a rifamycin derivative that inhibits DNA-dependent RNA polymerase - Clofazimine is a riminophenazine compound with antimycobacterial and anti-inflammatory properties The combination has shown efficacy in both in vitro biofilm assays and in vivo murine models of M. avium infection. The therapy is designed to be administered as one pill daily, which may provide superior patient adherence and potentially decrease the emergence of bacterial resistance during treatment.
