Overview

Rifabutin + efavirenz is a drug combination used in the treatment of patients with HIV and tuberculosis co-infection. This combination requires special dosing considerations due to drug-drug interactions. ## Description Rifabutin is an antibiotic derived from rifamycin that is primarily used to prevent and treat Mycobacterium avium complex (MAC) disease in patients with HIV[1][5]. It's also used as part of combination therapy for Helicobacter pylori infection[5]. Efavirenz is a non-nucleoside reverse transcriptase inhibitor (NNRTI) that works by inhibiting HIV's reverse transcriptase enzyme, preventing the virus from replicating[4][7]. It's used in combination with other antiretroviral medications to treat HIV infection. When used together, these medications require dose adjustments due to significant drug-drug interactions. Efavirenz induces the metabolism of rifabutin, necessitating an increase in rifabutin dosage from the standard 300 mg to 600 mg to maintain therapeutic levels[2][3]. ## Pharmacokinetic Interactions Studies have shown that when rifabutin (300 mg) is co-administered with efavirenz, rifabutin levels are significantly reduced due to efavirenz's effect on hepatic cytochrome P450 enzymes. Increasing the rifabutin dose to 600 mg has been found to adequately compensate for this interaction in most patients[3]. This dose adjustment helps maintain effective rifabutin concentrations while allowing patients to benefit from efavirenz-based antiretroviral therapy. Research has demonstrated that this adjusted dosing regimen is effective, with studies showing that HIV viral load decreased by 2.6 log copies/mL and median CD4+ T cell counts increased from 141 to 240 cells/mm³ after approximately 21 days of efavirenz-based therapy in patients also receiving rifabutin[3]. ## Clinical Use This combination is particularly important for patients with HIV who also have tuberculosis or who require prophylaxis against Mycobacterium avium complex. The dose adjustments allow for effective treatment of both conditions simultaneously, which is crucial for patient outcomes in these complex cases.
