Rifampin + midazolam is a combination of two pharmaceutical agents used primarily in research and clinical pharmacology to study drug-drug interactions. Rifampin is an antibiotic from the rifamycin class, mainly used to treat tuberculosis and eliminate *Neisseria meningitidis* carriers. It acts as a potent inducer of cytochrome P450 3A4 (CYP3A4) enzymes, leading to increased metabolism of drugs processed by this pathway[4]. Midazolam is a short-acting benzodiazepine with sedative, anxiolytic, muscle relaxant, anticonvulsant, and amnesic properties. Its effects are mediated through positive allosteric modulation of GABA-A receptors[5][6]. When administered together, rifampin significantly reduces plasma concentrations and pharmacodynamic effects of midazolam due to CYP3A4 induction by rifampin in both the gut wall and liver; this can render oral midazolam ineffective during concurrent treatment[1][2][6].
