Overview

**Rifapentine + clarithromycin** is a combination of two antibiotic drugs: rifapentine, a rifamycin-class antibiotic that inhibits bacterial DNA-dependent RNA polymerase, and clarithromycin, a macrolide-class antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. The combination has been investigated, primarily in preclinical and some clinical settings, as an all-oral regimen for the treatment of mycobacterial infections including Buruli ulcer (caused by Mycobacterium ulcerans) and non-tuberculous mycobacterial (NTM) infections such as those caused by the Mycobacterium avium complex and Mycobacterium kansasii[1][3]. Notably, co-administration may lead to clinically significant pharmacokinetic interactions, with clarithromycin reducing plasma rifapentine levels, potentially through CYP3A-mediated processes[2][3]. Neither a unique brand name nor a widely approved fixed-dose combination exists; regimens are constructed from the use of both drugs together.
