Overview

This combination therapy involves the sequential administration of rifaximin, a non-absorbable antibiotic, followed by fecal microbiota transplantation (FMT) for the treatment of diarrhea-predominant irritable bowel syndrome (IBS-D). Rifaximin acts by binding to the beta-subunit of bacterial DNA-dependent RNA polymerase, thereby inhibiting bacterial RNA synthesis and reducing the overall bacterial load or specific dysbiotic strains in the gastrointestinal tract. Following this decontamination phase, FMT is performed to introduce a diverse community of healthy commensal microorganisms from a donor, aiming to restore a balanced gut microbiome and alleviate symptoms such as abdominal pain and diarrhea. This specific regimen is being evaluated by Shanghai Zhongshan Hospital in clinical trials.
