Overview

rifaximin + omeprazole + amoxicillin + clarithromycin is a quadruple therapy regimen being investigated for the eradication of Helicobacter pylori infection. This combination adds rifaximin, a non-absorbed rifamycin antibiotic that inhibits bacterial DNA-dependent RNA polymerase, to the standard triple therapy consisting of omeprazole (a proton pump inhibitor that inhibits the H+/K+-ATPase pump), amoxicillin (a beta-lactam antibiotic that targets penicillin-binding proteins), and clarithromycin (a macrolide antibiotic that binds to the 50S ribosomal subunit). The regimen is designed to improve eradication rates, particularly in patients with clarithromycin-resistant H. pylori strains, by utilizing the broad-spectrum activity and low systemic absorption of rifaximin. It has been evaluated in a Phase 3 pilot clinical trial sponsored by the Fundación de Investigación Biomédica - Hospital Universitario de La Princesa.
