This combination therapy consists of rimegepant, a small molecule calcitonin gene-related peptide (CGRP) receptor antagonist, administered alongside the standard-of-care chemotherapy regimen of nab-paclitaxel and gemcitabine (AG). Rimegepant is traditionally used for the treatment of migraine, but in this oncology context, it is being investigated for its potential to modulate the tumor microenvironment or neurogenic inflammation in pancreatic cancer. Nab-paclitaxel is an albumin-bound formulation of paclitaxel that inhibits microtubule dynamics, while gemcitabine is a nucleoside analog that interferes with DNA synthesis. The combination is currently being evaluated in a Phase Ib/II clinical trial led by Xiuchao Wang as a first-line treatment for patients with metastatic pancreatic ductal adenocarcinoma (mPDAC).
