Ringer's sodium pyruvate solution is a modified Ringer’s solution in which the anion component includes sodium pyruvate instead of, or in addition to, other anions such as lactate. It is used as an intravenous resuscitation fluid to promote blood circulation, correct acidosis, and maintain acid-base equilibrium during critical illness and shock states. The mechanism of action involves providing metabolic substrate (pyruvate) that can be rapidly metabolized to support cellular energy production and act as a potent systemic alkalizer by consuming hydrogen ions through enzymatic reactions (notably via lactate dehydrogenase), thereby correcting lactic acidosis. Clinical studies suggest it can reduce inflammation (lowering TNF-α and IL-6), improve hemodynamics, protect intestinal barrier function, enhance free radical scavenging capacity, and lower mortality rates in septic shock patients[2][5]. Its primary indication is for use in severe metabolic lactic acidosis and resuscitation scenarios.
