Ringer's lactate + albumin is a combination fluid resuscitation strategy used in the management of critically ill patients, particularly those in septic shock. Ringer's lactate is a balanced crystalloid solution containing sodium chloride, sodium lactate, potassium chloride, and calcium chloride, designed to maintain electrolyte balance and provide buffering through lactate metabolism. Albumin is a natural plasma protein that provides approximately 75-80% of the normal plasma colloid oncotic pressure. The co-administration of these two agents aims to optimize intravascular volume expansion while minimizing the risks of hyperchloremic metabolic acidosis and acute kidney injury associated with normal saline. Clinical data, such as that from the MIMIC-IV database, suggests that this combination may be superior to saline or Ringer's lactate alone in reducing in-hospital mortality and improving renal outcomes in septic shock patients.
