Ringer Lactate + norepinephrine is a combination of two medical products used in clinical settings, particularly for patients with hypotension or in shock states. This combination consists of Ringer's lactate solution (an intravenous fluid) and norepinephrine (a vasopressor medication). ## Description Ringer Lactate (also known as Lactated Ringer's, RL, or Hartmann's solution) is an isotonic crystalloid fluid used for fluid resuscitation and electrolyte replacement. It contains sodium chloride, sodium lactate, potassium chloride, and calcium chloride in water[6]. The solution is isotonic with blood and has a pH of about 6.5[8]. Norepinephrine (also known as Levophed) is a sympathomimetic vasopressor that stimulates alpha-adrenergic receptors to produce vasoconstriction and increase blood pressure. At lower doses (<2 mcg/min), it also stimulates beta-adrenergic receptors to enhance myocardial contractility[4][5]. It is the first-line pharmacological agent for treating hypotension, particularly in vasodilatory shock states such as septic shock and neurogenic shock[5]. The combination is used in critical care settings where both fluid resuscitation and vasopressor support are needed simultaneously. ## Administration Norepinephrine can be administered as an IV infusion in Ringer's Lactate solution. The typical preparation is 8 mg of norepinephrine in 250 mL of Ringer's Lactate (32 mcg/mL)[4]. This combination allows for both fluid replacement and blood pressure support in critically ill patients. ## Compatibility According to compatibility studies, norepinephrine is physically compatible with Ringer's Lactate solution for intravenous administration[4][7]. This compatibility allows for the simultaneous delivery of both components through the same intravenous line, which can be particularly useful in emergency and critical care settings. ## Clinical Considerations When administering this combination: - Hypovolemia, hypoxemia, and acidosis should be corrected concurrently with initiation of therapy[4] - The norepinephrine component should be weaned off gradually - Close monitoring of blood pressure is essential - The solution may change color from clear to brown, indicating a loss of potency of the norepinephrine component[4] ## Precautions - Inadvertent boluses of norepinephrine may cause profound hypertension, potentially resulting in myocardial infarction or cerebral ischemia/bleeding[4] - Norepinephrine alone without inotropic therapy may worsen stroke volume in patients with left ventricular dysfunction and poor ejection fraction[4] - Potential adverse effects include bradycardia, cardiac arrhythmias, chest pain, renal ischemia, mesenteric ischemia, and hyperglycemia[4] - Norepinephrine is a vesicant which can cause progressive and severe tissue damage if extravasation occurs[4]
