Drug intelligence / Profile preview
This is a combination therapy consisting of two intravenous fluids (Ringer's lactate and hydroxyethyl starch) and two cardiovascular medications (norepinephrine and dobutamine). This combination may be used in the management of septic shock and other forms of shock. **Components:** 1. **Ringer's lactate**: An isotonic crystalloid solution used for fluid resuscitation that contains sodium lactate, which can help correct metabolic acidosis. 2. **Hydroxyethyl starch**: A synthetic colloid volume expander used to maintain oncotic pressure and expand plasma volume. Available as products like Voluven (6% hydroxyethyl starch 130/0.4) or Hextend (6% hetastarch in lactated electrolyte injection). 3. **Norepinephrine**: A vasopressor that increases blood pressure through vasoconstriction, primarily used in hypotensive states. 4. **Dobutamine**: An inotropic agent that increases cardiac output by enhancing myocardial contractility without significant vasoconstriction. In septic shock management, Ringer's lactate and hydroxyethyl starch may be used for initial fluid resuscitation, followed by norepinephrine to maintain blood pressure, with dobutamine added to improve cardiac output in patients with myocardial dysfunction.
