risperidone + ramelteon

This is a combination of two pharmaceutical drugs, risperidone and ramelteon. Risperidone is an atypical antipsychotic primarily used to treat schizophrenia, bipolar disorder, and irritability associated with autism. It acts mainly as an antagonist at dopamine D2 receptors and serotonin 5-HT2A receptors, reducing psychotic symptoms by modulating neurotransmitter activity in the brain. Ramelteon is a melatonin receptor agonist indicated for insomnia characterized by difficulty with sleep onset; it selectively binds to melatonin MT1 and MT2 receptors in the suprachiasmatic nucleus of the brain, promoting sleep initiation without significant affinity for GABA or other CNS targets. The combination may be considered in patients with schizophrenia who experience comorbid sleep or circadian rhythm disturbances, as add-on ramelteon can improve sleep quality without exacerbating psychosis[1][4]. Additive central nervous system (CNS) depressant effects are possible when these agents are combined[4][9].
