Overview

This entry refers to the combination of three direct oral anticoagulants (DOACs): rivaroxaban, dabigatran, and apixaban. These agents are not used together as a fixed-dose combination or co-formulated product; rather, they are individual drugs within the same therapeutic class. All three are approved for prevention of stroke and systemic embolism in patients with non-valvular atrial fibrillation (AF), treatment and secondary prevention of venous thromboembolism (VTE), and thromboprophylaxis after major orthopedic surgery[6][8]. - **Rivaroxaban** is a direct Factor Xa inhibitor. - **Apixaban** is also a direct Factor Xa inhibitor. - **Dabigatran** is a direct thrombin (Factor IIa) inhibitor[8]. These drugs act by selectively inhibiting key enzymes in the coagulation cascade, thereby reducing clot formation. They have more predictable pharmacokinetics than warfarin and do not require routine INR monitoring[8]. Comparative studies show similar effectiveness among these agents for stroke prevention in AF; however, apixaban generally has the most favorable safety profile with lower risks of major bleeding compared to rivaroxaban or dabigatran[1][2][3][5][7][9].
