Overview

Rivaroxaban + unfractionated heparin is a combination of two anticoagulant drugs used to prevent and treat thromboembolic disorders. Rivaroxaban is an oral, direct factor Xa inhibitor that blocks both free and clot-bound factor Xa, thereby inhibiting the conversion of prothrombin to thrombin and ultimately preventing fibrin clot formation. It does not require antithrombin III for its action and has predictable pharmacokinetics[1][6]. Unfractionated heparin (UFH) is a parenteral anticoagulant that acts indirectly by binding to antithrombin III, which then inactivates several coagulation factors including factor Xa and thrombin. This combination may be used during transitions between parenteral (UFH) and oral (rivaroxaban) anticoagulation or in specific clinical scenarios requiring overlapping therapy[3]. Rivaroxaban was developed by Bayer; Xarelto is marketed by Bayer and Johnson & Johnson.
