A combination immunotherapy consisting of the recombinant bispecific single-chain antibody rM28 and autologous peripheral blood mononuclear cells (PBMCs). Developed by the University Hospital Tuebingen, this therapy is designed for the intralesional treatment of metastatic stage III/IV malignant melanoma. The rM28 antibody is engineered to target the melanoma-associated chondroitin sulfate proteoglycan (NG2/CSPG4) on tumor cells and the costimulatory CD28 molecule on T cells. By bridging these two, it induces T-cell activation and subsequent tumor cell killing. Notably, this activation is dependent on the presence of NG2-positive cells and does not require a primary signal through the T-cell receptor (TCR)/CD3 complex, effectively bypassing the need for MHC-restricted antigen presentation to trigger an immune response.
