rNAPc2 + unfractionated heparin is a combination therapy comprising recombinant nematode anticoagulant protein c2 (rNAPc2) and unfractionated heparin. rNAPc2 is a recombinant protein isolated from a haematophagous hookworm and acts as a potent, selective inhibitor of the tissue factor (TF)/activated factor VII (FVIIa) complex, which initiates blood coagulation. Its mechanism of action involves prerequisite binding to circulating activated factor X (FXa) or zymogen factor X (FX) prior to interaction and inhibition of the FVIIa/TF complex[4]. rNAPc2 has demonstrated anticoagulant, anti-inflammatory, and potential antiviral properties[2]. Unfractionated heparin is a widely used anticoagulant that enhances the activity of antithrombin III, leading to inhibition of thrombin and other clotting factors. The combination has been studied for conditions with thrombotic risk, including acute coronary syndromes and COVID-19[3][2], where synergism of anticoagulant mechanisms may be beneficial.
