RO5083945 + FOLFIRI is an experimental combination regimen comprising RO5083945, a humanized monoclonal antibody targeting CD19 (developed by Roche), and FOLFIRI, a standard chemotherapeutic regimen containing folinic acid (leucovorin), fluorouracil (5-FU), and irinotecan. FOLFIRI is commonly used for metastatic colorectal cancer and other gastrointestinal malignancies. The mechanism of FOLFIRI involves the inhibition of DNA synthesis and cell division through several mechanisms: 5-FU disrupts nucleotide metabolism and irreversibly inhibits thymidylate synthase, leading to DNA and RNA damage; irinotecan inhibits topoisomerase I, preventing DNA replication; and folinic acid potentiates the effects of 5-FU. RO5083945, as an anti-CD19 monoclonal antibody, binds to B-cell-specific CD19, mediating B-cell depletion, though this mechanism is not relevant to standard colorectal cancer therapy. This combination is investigational and not an established standard of care.
