**Roflumilast + alogliptin** is a combination of two small molecule drugs: roflumilast, a selective phosphodiesterase-4 (PDE4) inhibitor, and alogliptin, a selective dipeptidyl peptidase-4 (DPP-4) inhibitor. Roflumilast inhibits PDE4, leading to an accumulation of intracellular cyclic AMP and downregulation of inflammatory responses, and is primarily used to reduce risk of exacerbations in severe chronic obstructive pulmonary disease (COPD)[2][3]. Alogliptin inhibits DPP-4, prolonging the action of incretin hormones like GLP-1, which increases insulin synthesis and decreases glucagon secretion, thereby improving blood glucose control in type 2 diabetes. The combination has been studied in a proof-of-mechanism trial to evaluate their effect on postprandial active GLP-1 and 24-hour glucose levels in patients with type 2 diabetes inadequately controlled with metformin[1].
