Romiplostim + decitabine is a combination of two drugs used primarily in clinical studies for patients with myelodysplastic syndromes (MDS) and other hematologic disorders characterized by thrombocytopenia. Romiplostim is a peptibody thrombopoietin receptor (TPO-R or MPL) agonist that stimulates megakaryocyte maturation and increases platelet production. Decitabine is a hypomethylating agent that incorporates into DNA and inhibits DNA methyltransferase, leading to DNA hypomethylation, reactivation of silenced genes, and induction of cell differentiation or apoptosis, especially in leukemia and MDS. The rationale for combining these agents is to manage decitabine-induced thrombocytopenia by stimulating platelet production with romiplostim, without compromising the antineoplastic effects of decitabine[1][7]. The main mechanism is a combination of stimulating platelet production while modulating epigenetic activity for antineoplastic effects.
