Drug intelligence / Profile preview
Nplate + prednisone is a combination regimen consisting of romiplostim (Nplate), a thrombopoietin (TPO) receptor agonist, and prednisone, a synthetic glucocorticoid corticosteroid. Romiplostim is a recombinant Fc-peptide fusion protein (peptibody) that binds to and activates the TPO receptor (c-Mpl) on megakaryocytes, stimulating intracellular signaling pathways (primarily JAK2/STAT5) to increase platelet production. Prednisone is a small-molecule glucocorticoid receptor agonist that, upon metabolic conversion to prednisolone, suppresses the immune system and reduces the autoimmune destruction of platelets. This combination is primarily investigated for the treatment of primary immune thrombocytopenia (ITP), particularly in newly diagnosed or previously untreated adult patients, to rapidly elevate platelet counts and achieve sustained remission while minimizing long-term corticosteroid exposure. Amgen is evaluating this combination in the Phase 3 ROMISTER trial.
