Overview

This multimodal local anesthetic cocktail was developed by the Lahey Clinic for perioperative pain management in patients undergoing operative fixation of ankle fractures. The formulation consists of ropivacaine (a long-acting amide local anesthetic), clonidine (an alpha-2 adrenergic agonist used as an analgesic adjuvant), and epinephrine (a vasoconstrictor used to prolong the duration of the block). It is administered via local infiltration at the surgical site. The cocktail is being evaluated in a Phase 4 clinical trial (NCT03696199) to compare its efficacy against regional anesthesia (nerve blocks) and traditional care. The combination aims to provide extended regional pain control by blocking nerve conduction while minimizing systemic absorption and enhancing the analgesic effect through synergistic pathways.
