Overview

This is a combination of three agents commonly used in regional anesthesia and pain management: - **Ropivacaine** is an amide-type local anesthetic that blocks voltage-gated sodium channels, inhibiting nerve impulse conduction and providing regional anesthesia or analgesia. - **Epinephrine** (adrenaline) is a vasoconstrictor that, when added to local anesthetics, reduces systemic absorption, prolongs the duration of action at the injection site, and decreases bleeding. - **Dexamethasone** is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory properties. When used as an adjuvant in nerve blocks or local infiltration, it can significantly prolong the duration of analgesia. The combination aims to provide prolonged and enhanced pain relief for procedures such as peripheral nerve blocks or spinal injections. Dexamethasone has been shown to increase block duration when added to ropivacaine; however, epinephrine does not further enhance this effect[2][7]. There are reports of potential incompatibility between ropivacaine and dexamethasone due to pH-dependent crystallization[1], which may pose risks if injected intravascularly. This mixture should be prepared with caution.
