Overview

This is a combination of three agents—ropivacaine, sodium bicarbonate, and dexamethasone—used primarily as an admixture for regional anesthesia, such as peripheral nerve blocks. Ropivacaine is an amide-type local anesthetic that works by blocking voltage-gated sodium channels in neuronal membranes, thereby inhibiting nerve impulse transmission and providing analgesia and anesthesia. Sodium bicarbonate acts to alkalinize the solution, increasing the proportion of nonionized (active) ropivacaine molecules; this can hasten onset and potentially prolong block duration but also increases risk of precipitation. Dexamethasone is a synthetic glucocorticoid corticosteroid added as an adjuvant to further prolong the duration of sensory and motor block through anti-inflammatory effects and possible direct action on potassium channels in nerves[1][4][8]. The combination has been shown to provide faster onset and longer-lasting analgesia compared to ropivacaine alone or with only one adjuvant[1]. However, there are significant concerns about rapid crystallization/precipitation when these drugs are mixed due to pH incompatibility between ropivacaine (which is less stable at higher pH) and both sodium bicarbonate (alkaline) and dexamethasone solutions. This precipitation may pose risks if injected intravascularly or near neural tissue[2][3][4].
