RU38486 + spironolactone is an experimental combination of two small molecule drugs, mifepristone (also known as RU38486) and spironolactone. Mifepristone is a glucocorticoid receptor (GR) antagonist, while spironolactone is a mineralocorticoid receptor (MR) antagonist. This combination has been studied in research settings to investigate the roles of GR and MR in hypothalamic-pituitary-adrenal (HPA) axis regulation, particularly in conditions such as obesity and cognitive function. The combined blockade of both receptors leads to substantial elevation of cortisol concentrations by disrupting negative feedback on the HPA axis, revealing redundancy between MR and GR in this feedback mechanism[1][4]. This approach has been used experimentally to dissect abnormal HPA axis control relevant to neuropsychiatric disorders, metabolic syndrome, obesity, depression, and cognitive decline[1].
