Rucaparib + nivolumab is an investigational combination therapy studied for maintenance and treatment of several cancers, primarily advanced ovarian cancer, metastatic castration-resistant prostate cancer (mCRPC), and advanced biliary tract cancer. Rucaparib is an oral small-molecule inhibitor of poly(ADP-ribose) polymerase (PARP), which is involved in DNA damage repair, leading to synthetic lethality in tumor cells with homologous recombination deficiency. Nivolumab is an intravenous monoclonal antibody that targets and inhibits the programmed death-1 (PD-1) receptor, enhancing anti-tumor immune responses by blocking tumor-mediated immune evasion. The combination has been evaluated to improve progression-free survival by synergistically increasing immune-mediated tumor cell death, but pivotal phase 3 studies such as ATHENA-COMBO have not met their primary efficacy endpoints for maintenance therapy in ovarian cancer. The regimen continues to be explored in other solid tumors, mainly as a post-chemotherapy maintenance or in homologous recombination-deficient settings[1][2][4][6][7].
