RV4421A + BS0042 is a topical combination therapy developed by Pierre Fabre Dermo-Cosmétique for the management of atopic dermatitis. The product combines two proprietary active ingredients: RV4421A, a flavonoid- and saponin-rich extract from Rhealba Oat (*Avena sativa*) plantlets, and BS0042, a biotechnological extract derived from the bacterium *Aquaphilus dolomiae* (also known as I-modulia). RV4421A provides anti-inflammatory and skin-barrier-restoring properties by inhibiting the production of pro-inflammatory mediators such as thymic stromal lymphopoietin (TSLP) and interleukin-8 (IL-8). BS0042 acts as a selective antagonist of protease-activated receptor 2 (PAR-2), a key mediator of pruritus and inflammation in the skin. This combination is primarily investigated as an adjuvant to topical corticosteroids, such as desonide, to accelerate the reduction of clinical symptoms like the Local SCORAD (L-SCORAD) index and to provide a steroid-sparing effect in patients with atopic dermatitis.
