RXC007 + pirfenidone is a combination therapy of two distinct small molecules with anti-fibrotic properties, considered for treatment of idiopathic pulmonary fibrosis (IPF). RXC007 (also known as zelasudil) is a selective oral inhibitor of Rho-associated protein kinase 2 (ROCK2), disrupting key pro-fibrotic and inflammatory signaling pathways, and has shown anti-fibrotic efficacy in preclinical and clinical studies. Pirfenidone is an approved anti-fibrotic small molecule that inhibits myofibroblast proliferation, reduces the production of fibrogenic mediators, and is standard-of-care for IPF. The rationale for the combination is to enhance anti-fibrotic efficacy through distinct and potentially complementary mechanisms. RXC007 is developed by Redx Pharma and is in phase 2 clinical studies as monotherapy in IPF; formal combination studies with pirfenidone may be explored or considered for enhanced efficacy, but direct combination trial status should be checked for updates.
