S-1 + bevacizumab + pemetrexed is a combination chemotherapy regimen used primarily in the treatment of advanced non-squamous non-small cell lung cancer (NSCLC). - **S-1** is an oral fluoropyrimidine derivative that combines tegafur (a prodrug of 5-fluorouracil), gimeracil, and oteracil to enhance antitumor activity and reduce gastrointestinal toxicity. It acts mainly by inhibiting thymidylate synthase, thereby interfering with DNA synthesis in rapidly dividing tumor cells. - **Bevacizumab** is a recombinant humanized monoclonal antibody targeting vascular endothelial growth factor A (VEGF-A), inhibiting angiogenesis required for tumor growth and metastasis. - **Pemetrexed** is a multi-targeted antifolate chemotherapeutic agent that inhibits several folate-dependent enzymes involved in purine and pyrimidine synthesis, including thymidylate synthase. This triple combination has been investigated as a first-line or maintenance therapy for advanced NSCLC, with some studies suggesting it may offer improved disease control rates compared to doublet regimens[7]. However, increased toxicity has also been reported with the addition of multiple agents.
