S-1 + carboplatin is a combination chemotherapy regimen consisting of the oral fluoropyrimidine derivative S-1 and the platinum-based alkylating agent carboplatin. This combination is primarily used in the treatment of advanced or recurrent non-small-cell lung cancer (NSCLC), including in patients with comorbid interstitial lung disease (ILD). S-1 is an oral anticancer agent composed of tegafur (a prodrug of 5-fluorouracil), gimeracil (an inhibitor of dihydropyrimidine dehydrogenase to increase 5-FU bioavailability), and oteracil (to reduce gastrointestinal toxicity). It acts as an antimetabolite by inhibiting thymidylate synthase, thereby interfering with DNA synthesis in rapidly dividing cells. Carboplatin is a platinum-based chemotherapeutic that forms DNA crosslinks, leading to apoptosis in cancer cells. The combination has demonstrated noninferior overall survival compared to standard regimens such as paclitaxel plus carboplatin for advanced NSCLC[2][1]. The regimen typically involves administration every 3–4 weeks, with specific dosing based on body surface area and renal function.
