S-1 + cisplatin is a combination chemotherapy regimen primarily used for the treatment of advanced gastric and gastroesophageal adenocarcinoma. S-1 is an oral fluoropyrimidine derivative designed to improve the efficacy and tolerability of 5-fluorouracil (5-FU) by combining tegafur (a prodrug of 5-FU) with two modulators that enhance its activity and reduce gastrointestinal toxicity. Cisplatin is a platinum-based alkylating agent that exerts its antineoplastic effects by forming DNA cross-links, thereby inhibiting DNA replication and transcription which leads to cell death. The combination has demonstrated noninferior overall survival compared to infusional 5-FU/cisplatin regimens with a better safety profile in phase 3 clinical trials for advanced gastric cancer[2][3][5][7].
