TIROX is a triplet chemotherapy regimen combining three active cytotoxic agents: S-1 (an oral fluoropyrimidine), irinotecan (a topoisomerase I inhibitor), and oxaliplatin (a platinum-based agent). Developed and studied by the National Cancer Center in Korea, this combination is primarily investigated for the treatment of advanced or metastatic gastrointestinal malignancies, including colorectal and gastric cancers. S-1 is a multi-component drug consisting of tegafur (a 5-fluorouracil prodrug), gimeracil (a dihydropyrimidine dehydrogenase inhibitor), and oteracil potassium (which reduces gastrointestinal toxicity). Irinotecan works by inhibiting DNA topoisomerase I, preventing DNA religation and causing double-strand breaks, while oxaliplatin forms DNA cross-links that inhibit replication and transcription. Clinical trials have demonstrated high objective response rates, particularly in gastric cancer, with a manageable safety profile.
